A:  Yes, the coach could have the skin form on a tablet as long as it is completed properly and can be reviewed by the referee or the appropriate health-care professional.  Now keep in mind, if the appropriate health-care professional requests a hard copy to keep on file at the meet, the coach would have to produce a hard copy to give to the AHCP via printing.

At both state tournaments, a hard copy is required.
